The starting
point

In August 2018, Google rolled out an algorithm update that hit health websites harder than anyone else: the so-called Medic Update. Overnight, the bar for ranking medical content got higher. Google started demanding concrete proof of expertise, authoritativeness and trustworthiness (E-A-T) before showing a page among the top results. For a New York fertility clinic with more than 2,000 pages of medical content, the challenge was twofold: protecting their visibility in this new landscape, and bringing order to a massive content library so that every single page answered exactly what patients were searching for.

Getting to
work

We tackled the problem on two fronts. First, authority: we added author biography boxes to the medical articles, so both Google and patients could verify who stood behind every piece of content, and we implemented structured data (Schema markup) to help search engines make better sense of each page. In parallel, we built out and optimized the clinic's local profiles on Google, Bing and Apple Maps, with a local strategy squarely focused on New York users.

Second, content architecture: working side by side with the client's team, we defined 11 topic clusters representing both their business areas and the way patients actually search. Starting from roughly 100 seed keywords, we expanded the research to 16,055 keywords, adding up to more than 14.7 million monthly searches. We prioritized by volume — the top cluster alone accounted for nearly 9.7 million searches a month. Then, page by page, we assigned each of the site's 2,140 URLs a main keyword and a set of secondary ones, optimizing URL, title, H1, H2, content and internal links.

Method made the difference. We ran everything on a shared, collaborative platform with the client's team, where every optimization was proposed, discussed and approved before going live. We even built a tool of our own: a keyword lookup that checked every term against the site's complete URL database, instantly flagging whether a page was already optimized for that search — and keeping cannibalization at bay. And we were surgical with what was already working: pages that ranked well received moderate tweaks so we wouldn't put existing traffic at risk, while deep optimizations were reserved for the pages with the most room to grow.

result

The strategy turned the site into a reference in its specialty. The page optimized for their key search term — 12,100 monthly searches — climbed from position 5 to position 1 on Google. Brand-new content, created from scratch out of the cluster research, reached the first page of results. And the site was left fully prepared for the E-A-T standard Google now demands of medical content: verifiable authors, trustworthy information, and an architecture where every page answers a concrete patient question.
